CONSENT TO TREAT MINORS!
When your child or children are coming in for a visit, whether it is for a sick visit, well check-up or vaccinations and you, the parent or legal guardian, will not be coming with them, we MUST have a signed consent form allowing our office to treat your child. In order to make it as easy as possible for you, we have attached the consent form for you to download, fill out and sign. You can return it to our office by fax at 805-461-0361.

IMPORTANT INFORMATION!!!
Parents, some of you may or may not know, but our nurse practitioner, Ellen, is now on a medical leave. Below is a letter to you, our parents and patients, that Ellen wanted to share:

To our Patients,
I will be out of the office on medical leave for the next 6-7 months. I was recently diagnosed with
breast cancer and will be receiving treatment - for a cure!!
(Ladies, get your yearly gynecologic exam and mammogram if you are of the age!)
I look forward to returning in the Spring to the patients and families and the job that I love.
Ellen
P.S. Ellen is looking forward to returning June 1, 2012, but we will keep you posted! Thank you so much for your support!

NEW FORMS!!
With the beginning of each new year, we have to ask all of our patients to fill out new forms. Please click on the link to each of these forms to download and print. Have them filled out before your appointment and your check-in time will be a breeze!! Thank you for your cooperation.

PLEASE EXCUSE OUR DUST!! We are currently renovating our website and patient portal so for the time being you can no longer access your child's immunization records or make a payment from our website. If you want to make a payment on-line, please go to our secure website at https://pay.instamed.com. Our provider ID is pat.ted.peds.
Thank you for your patience while we work on a new and improved website which we hope to have up and running in March 2012!!

New vaccine requirements for school!!
State law is now requiring Tdap vaccines to all children heading into Jr. High. Please make sure that all of your child's vaccines are up to date. Click on the link below to see more information on vaccine requirements.
A DOZEN FUN FAMILY RITUALS

JUST A REMINDER:
As of June 1st, 2011, we no longer accept CenCal/MediCal insurance.

A NEW OFFICE POLICY STARTING JUNE 1, 2010
We are implementing a new office policy starting June 1, 2010, regarding the filling out of various forms. Due to the increase in staff hours for completing school, camp, and sports forms; home health and home therapy forms; letters for school and PE absences; and professional letters we have found it necessary to start charging a nominal fee for completing such forms and letters. The new fees are as follows:

One-page forms (school, camp, etc.): $5.00
Multi-page forms (school, camp, etc.): $10.00
Professional business letter: $20-$100 (based on time)

We require a 48-hour interal to fill out forms. You will be given an invoice and will need to pay for the provider's services at the time you pick up the forms. These charges will NOT BE APPLIED to forms that are presented during an actual office visit or physical exam.
Thank you for your understanding and please feel free to contact our office if you have any questions.

Be sure to use our WebPortal to access information on visits, immunizations, growth charts and billing.
